What are the different question types?

This article explains the different question types available and how to customize each question type.

1. Contact Information

Contact information has all of the standard contact information fields included.

2. Address

Address only has fields that are specific for obtaining an address.

3. Email

Email only has an email field. The value has to be in a "example@domain.com" format for the submission to be accepted.

4. Phone

Phone adds only one phone field.

5. Website

Website adds a field used to input a website

6. Date and Time

Date and Time adds a field to collect a date and time.

7. Text

Text has two options on the size of the text field:  long text fields (multiple lines of text) or single text lines. When a text question block is added, 12 single lines of text appear, but you can unselect unnecessary text fields and modify them to be long text. All 12 fields do not have to be the same. Some may be long text areas and some may be single text lines.

The question or header is for the entire question block section. To modify the question for individual lines of text, see step 7.2.

Group formatting style allows you to decide how you would like the lines of text displayed.

  • Standard - slight space between each text line with the field label above each text line
  • Columns - the text lines will be shown in 3 columns with the field label above each text line
  • Compact - no space between each text line with the field label to the left of the text line

Uncheck the box next to the lines of text that you do not wish to appear. Example: if you need 10 lines of text, uncheck Text11 and Text12.

  • Field Label - the header for the question
  • Placeholder Text - the text that is shown within the question field before a value is entered
  • Default Value - the answer for that field regardless if a value is typed in by the user
  • Field is Required - if checked, the field has to be populated for the form to be submitted
  • Field Size:    - Single Line: only used for short text answers    - Multiple Lines: used for longer text answers

8. Checkbox

Checkbox allows for you have multiple answers for one question that a customer can choose from. You can set the question to restrict the customer from only selecting one answer or multiple answers.

8.1. Click the large pencil to modify the question header, instructions or information, how many options can be selected, and the options themselves

Adding a minimum choice will make the question required. Maximum choices is the most options that someone can select.

To add an option:

  1. Click "Add Option"
  2. Input the answer/value in the "Display Name" field
  3. The answer/value will automatically input into the "Value" field

9. Document

Document allows users to take a picture or attach an existing picture to a submission. When this field type is added, you have the option for 5 attachments.

Uncheck the boxes within the visible fields to only show the number of document options that you wish to have available.

10. Dropdown

Dropdown allows users to pick an answer from a dropdown question type. You can only select one answer within the dropdown.

To add an option:

  1. Click "Add Option"
  2. Input the answer/value in the "Display Name" field
  3. The answer/value will automatically input into the "Value" field

11. Radio Button

Radio Button is great for true/false questions.

  1. Click "Add Option"
  2. Input the answer/value in the "Display Name" field
  3. The answer/value will automatically input into the "Value" field

12. Number

Number allows users to submit a numerical value. You can format the answer to have a numerical symbol associated. ($,%,#...) When a number question block is added, there are 12 fields that are automatically added, but you can remove unwanted fields.

The question or header is for the entire question block section. To modify the question for individual fields, see step 12.2.

  • Standard - slight space between each number line with the field label above each number line
  • Columns - the number lines will be shown in 3 columns with the field label above each number line
  • Compact - no space between each number line with the field label to the left of the number line

Uncheck the box next to the lines of numbers that you do not wish to appear. Example: if you need 10 lines of number fields, uncheck Number11 and Number12.

12.2. Click the small pencil to modify the specifications for individual number fields

  • Field Label - the header for the question
  • Placeholder Text - the text that is shown within the question field before a value is entered
  • Default Value - the answer for that field regardless if a value is typed in by the user
  • Symbol - the numerical symbol associated to that field
  • Symbol Placement - determine where the symbol is located compared to the answer
  • Field is Required - if checked, the field has to be populated for the form to be submitted
  • Only Accept Whole Numbers - submission will not be accepted if there is a decimal within the answer

13. Hidden Field

Hidden Field is used to add a label(s) to the form submission that is used within your CRM or marketing automation software. The customers will never see this field. You predetermine the value for this field. There are 5 hidden fields available within the question block. There is no way to remove any of the fields.

14. Section

Section adds a space between questions blocks for visual grouping.

15. Form Page Break

Form Page Break adds pagination to your form. The questions before Form Page Break will appear on one page. The questions after Form Page Break will appear on another page. You can create as many pages as necessary for your form.
